Patient Responsibilities

Nutrition counseling is a collaborative process and requires participation from patients. When patients are under age 18, family members, caregivers, and other household members involved in the patient’s care are requested to uphold these responsibilities as well. 

  • Be honest about facts, feelings, or ideas that relate to your care.
  • Take an active role in your treatment planning and therapy.
  • Report changes in your condition to your clinician.
  • Inform your clinician if you anticipate problems in following your treatment plan.
  • Ask for clarification if you do not understand issues related to your care.
  • Honor the confidentiality and privacy of other patients.
  • Communicate concerns and complaints.
  • Adhere to The Maker’s Table, L.L.C.’s Electronic Communication Guidelines and Policy, Late Arrival and Cancellation Policy, and Payment Policy.
  • Supply all medical referrals and/or medical authorizations as required by your insurance carrier prior to all appointments if choosing to pursue insurance coverage of your appointments.
